On the provincial map, Laguna’s land mass forms a semi-circle so that its boundaries are established on four provinces: Rizal, Quezon, Batangas, and Cavite.

If you live or work in Laguna and need a copy of a PSA birth, marriage, or death certificate, or a CENOMAR (Certificate of No Marriage), you may set an appointment at the nearest PSA Civil Registry System (CRS) outlet outlet in your city or municipality. Laguna residents may choose from the following PSA CRS outlets nearest to its boundaries:

Batangas: PSA, Ground Floor Building C, Fiesta World Mall, Marawoy, Lipa City, Batangas
Quezon: 3rd Floor Pacific Mall, ML Tagarao Street, Brgy. 3, Lucena City, Quezon
Paranaque City: 5th Floor Ayala Mall, Manila Bay Diosdado Macapagal Blvd. cor Aseana Ave. Parañaque City, 1701 Metro Manila

Currently, all PSA offices require visitors to get a confirmed online appointment before coming into the office. Setting an appointment is quick, easy, and free. Just follow the steps below:

Log-on to the PSA Online Appointment System webpage.
Select Region IV-A or NCR as the Region (whichever outlet you choose to visit). If you chose Region IV-A, select the Batangas or Quezon outlets, and if you chose NCR, select Paranaque as the outlet. Hit the Next button.
Provide your personal information as required by the appointment setting page. You will receive a verification code in your email, and you need to key-in this code to proceed to the rest of the appointment setting steps.
Select the PSA certificate you will be requesting for. If you are requesting for another person, indicate the details of the owner of the certificate and your relationship to him/her.
Choose the date and time of your planned visit. The PSA CRS outlets in Lipa City, Lucena City and Pasay City accept applicants from Monday to Friday. The Lipa outlet is open from 7am to 3:30pm, while the Paranaque City outlet between 7am to 3pm. Grayed out slots mean these are fully booked.
A summary of all the information you submitted will be shown on the next page. Double-check your details, dates, and other information submitted and make sure that all are correct. Your name on the appointment must match the name on your IDs.
Your Appointment Slip will be sent to your email. You may print this out or save a copy on your mobile phone. You will be asked to show this to the guard before you are allowed entry into the building.
Be at the PSA CRS outlet at least 30 minutes before your appointment time. Bring your own pen, wear your face mask, and observe other health protocols while in the office premises.
Minors are not allowed to make an online appointment.

Log-on to PSAHelpline.ph and click the order now button.
Choose the PSA certificate type you wish to order and click on that. You may order your own PSA certificates, that of your children, or parents.
Provide your personal details and the details on the certificate you are requesting for. You also need to type of ID you will be showing to the courier upon delivery of your requested document.
Choose the purpose of your request and specify if there were legal proceedings previously done on the certificate you are requesting for. If the certificate underwent a legal proceeding and this is the first time you will be requesting for the annotated copy, you need to personally request for a copy at a PSA CRS outlet.
Provide your delivery details, email address, and mobile number.
Select the number of copies you wish to order and confirm that all the details in your order are correct.
Pay through any of the available payment channels and wait for your PSA certificate to be delivered to your address.

PSAHelpline.ph will only release the ordered documents to the owner or the authorized requesting party. You must be physically present at the address you indicated in your order to receive the documents. The courier will request to see your identification; without it, the document will not be released to you. You cannot assign another person to receive your orders for you. Just as in setting an appointment to visit a PSA CRS outlet, minors are not allowed to order for themselves.
