How to Get a PSA Birth Certificate in South Cotabato

There are two PSA Civil Registry System (CRS) outlets in Region 12 where individuals can get copies of PSA birth, marriage, death certificates and Certificate of No Marriage (CENOMAR). One is in General Santos City and another in Koronadal.

You need to get a confirmed appointment before visiting any of the above outlets. You may do this online through the CRS Appointment System. Setting an appointment is free of charge and can be done using a computer, laptop, or mobile phone.

Here’s how you can set an online appointment through the CRS appointment system:

Log-on to the PSA Online Appointment System webpage.
Select Region XII.
Provide your personal information as required by the appointment setting page. You will receive a verification code in your email, and you need to key-in this code to proceed to the rest of the appointment-setting steps.
Select the PSA certificate you will be requesting for. If you are requesting for another person, indicate the details of the owner of the certificate and your relationship to him/her.
Choose the date and time of your planned visit. Both outlets accept requests for copy issuance of PSA certificates from Monday to Friday, 8am to 4pm. Grayed out slots mean these are fully booked.
A summary of all the information you submitted will be shown on the next page. Double-check your details, dates, and other information submitted and make sure that all are correct. Your name on the appointment must match the name on your IDs.
Your Appointment Slip will be sent to your email. You may print this out or save a copy on your mobile phone. You will be asked to show this to the guard before you are allowed entry into the building.
Be at the PSA CRS outlet at least 30 minutes before your appointment time. Bring your own pen, wear your face mask, and observe other health protocols while in the office premises.
Minors are not allowed to make an online appointment.

PSA Certificate Home Delivery

You also have the option to order your PSA certificate online and have it delivered to your home. This, too, is an online application process that can be done through a desktop, laptop, or mobile phone. When you order online, you no longer need to get an appointment with the PSA or visit a PSA outlet; your certificates will be delivered by a courier to your address.

Here’s how:

Log-on to PSAHelpline.ph and click the order now button.
Choose the PSA certificate type you wish to order and click on that. You may order your own PSA certificates, that of your children, or parents.
Provide your personal details and the details on the certificate you are requesting for. You also need to type of ID you will be showing to the courier upon delivery of your requested document.
Choose the purpose of your request and specify if there were legal proceedings previously done on the certificate you are requesting for. If the certificate underwent a legal proceeding and this is the first time you will be requesting for the annotated copy, you need to personally request for a copy at a PSA CRS outlet.
Provide your delivery details, email address, and mobile number.
Select the number of copies you wish to order and confirm that all the details in your order are correct.
Pay through any of the available payment channels and wait for your PSA certificate to be delivered to your address.

PSAHelpline.ph will only release the ordered documents to the owner or the authorized requesting party. You must be physically present at the address you indicated in your order to receive the documents. The courier will request to see your identification; without it, the document will not be released to you. You cannot assign another person to receive your orders for you. Just as in setting an appointment to visit a PSA CRS outlet, minors are not allowed to order for themselves.
