Lost Philippine passport

Losing a passport is undeniably a distressing experience that can throw anyone into a state of anxiety. Regardless of whether the passport is currently valid or has reached its expiration date, the process of obtaining a replacement can prove to be significantly more stressful than the initial application. Beyond the inconvenience, a lost passport poses a potential threat to your data privacy and security, given that it contains critical personal information such as your full name, birthdate, and other identifying details. Timely reporting of the loss is not only crucial for expediting the replacement procedure but also plays a pivotal role in ensuring your safety and peace of mind.

To guide you through the process of getting a replacement for your lost passport, we have compiled a complete and comprehensive step-by-step guide on obtaining a new passport after losing yours. This guide aims to make the process as straightforward as possible, providing you with the necessary knowledge to navigate through this rather annoying circumstance with confidence and ease.

Lost Valid Passport

Applying for a replacement of a lost valid ePassport is treated as a new application, and this includes securing a confirmed online appointment with the Department of Foreign Affairs (DFA), just as you did when initially applying for your passport. The required IDs and documents for first-time passport applicants will similarly apply to the replacement process.

Requirements for Adult New Applications:

  1. Confirmed online appointment. You may set an appointment at https://www.passport.gov.ph/appointment
  2. Accomplished Application Form
  3. Original and photocopy of applicant’s PSA birth certificate

    • Married female applicants who used their married last name in the lost passport must also present an original and photocopy of their PSA marriage certificate.
    • If PSA copies are not available yet or are blurry (unreadable), you may present the Local Civil Registrar copies of the same.
  4. Present ANY of the following IDs that the DFA considers acceptable for passport application. Prepare the original ID as well as a clear photocopy.

    • Philippine Identification (PhilID)/ePhilID
    • Social Security System (SSS) card
    • Government Service Insurance System (GSIS) card
    • Unified Multi-purpose Identification (UMID) card
    • Land Transportation Office (LTO) Driver’s License (license cards expiring on April 24 to October 31, 2023 and said expired cards perforated during the renewal process are accepted together with the temporary license in paper format).
    • Professional Regulatory Commission (PRC) ID
    • Overseas Workers Welfare Administration (OWWA) E-card
    • Commission on Elections (COMELEC) Voter’s ID or Voter’s Certificate issued from COMELEC main office in Intramuros, Manila.
    • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
    • Senior Citizen ID
    • Airman License (issued August 2016 onwards)
    • Philippine Postal ID (issued November 2016 onwards)
    • Seafarer’s Record Book (SRB) or Seafarer’s Identity Document (SID) – must be issued February 2020 onwards and issued by the Maritime Industry Authority (MARINA)
    • Valid or latest passport (for renewal of passport)
    • Valid school ID
      • For minor applicants – If school ID is not applicable, Certificate of Enrolment with photo of minor and dry seal of school.
      • For adult applicants – School ID and Certificate of Registration

In certain instances, providing a photocopy of the lost passport’s data page may exempt the need for a PSA birth certificate. However, for added assurance, it is advisable to carry the original and photocopy of your PSA birth certificate.

  1. Police Report in English (original and photocopy)
  2. Notarized Affidavit of Loss (original and photocopy)

Like the standard passport application process, it is vital to be present at your chosen DFA branch on the designated date and time of your appointment, furnished with the necessary set of IDs and documents. Ensure compliance with the prescribed dress code by the DFA and promptly settle the applicable fees. For complete information on how to set an appointment and other important details when applying for a Philippine passport, read this blog: What To Prepare For Your DFA Passport Appointment.

In the case of lost valid ePassports, please note that an extra 15 calendar days will be incorporated into the clearing period.

Lost Expired ePassports

The procedure of reporting and obtaining a replacement for a lost, expired ePassport closely mirrors that of applying for a replacement for a lost, valid ePassport, with one key difference: there is no longer a requirement for a police report for expired lost passports. Instead, you only need to submit an Affidavit of Loss in addition to all the other IDs and documentary requirements mentioned earlier.

Helpful Reminders for Passport Holders

How to get a new passport after losing your new or expired one
  1. Photograph your passport: Take clear photos of your passport’s data page and store them in secure digital locations, such as a cloud service or encrypted files on your devices.
  2. Backup important documents: Make digital copies of other essential travel documents, like visas and IDs, and keep them alongside your passport photos for quick access in case of loss.
  3. Use a passport holder: Keep your passport in a designated holder to protect it from wear and tear. Choose a holder with RFID-blocking features to prevent unauthorized scanning of your passport information.
  4. Secure storage: When not in use, store your passport in a safe and secure location, such as a home safe or a locked drawer. Avoid leaving it in easily accessible places.
  5. Be mindful in public places: When traveling or in crowded areas, be vigilant about your belongings. Keep your passport in a secure pocket or crossbody bag to minimize the risk of theft.
  6. Know Embassy/Consulate contact information: Familiarize yourself with the contact details of the nearest Philippine embassy or consulate at your travel destination. This information can be crucial in case of passport loss or emergency situations.
